Nursing Home Transition: Long-term Care Options
Nursing Home Transition (NHT) is a state-wide initiative of the Pennsylvania Department of Aging to expand the range of long-term care opportunities beyond traditional nursing home care for both elderly consumers and consumers of all ages with disabilities.
Area Agency on Aging (AAA) staff conduct regular informational and counseling sessions at 61 Allegheny County nursing homes to make long-term care consumers aware of community-based care options. In cases where relocation back into the community is appropriate and desirable for the consumer, NHT staff work closely with the consumer, family caregivers and nursing home staff to provide the necessary resources and assistance throughout the transition process.
The NHT program is a consumer-inspired initiative based on the concept that consumers have the right to make informed decisions about their own lives and well-being. It is not intended as a replacement for nursing homes and the vital role that they fill. Not every consumer is able or willing to receive care outside of a nursing facility. Rather, NHT counseling and services are offered as another option for qualified consumers in making decisions about long-term care.
