Provide Health Care Coordination and Support Services for Medically-Fragile Children in Child Welfare 

Allegheny County is soliciting proposals for the development and implementation of a Health Care Support and Coordination (HCSC) Pilot Program in three of its five Office of Children, Youth, and Families regional offices. The purpose of the HCSC Pilot Program is to improve the coordination of and communication regarding health care assessment and treatment for medically fragile children and youth involved in the child welfare system through consultation, collaboration and training.
