AAA Aging Waiver Service Coordination Program
The DHS Area Agency on Aging (AAA) Aging Waiver Service Coordination Program provides older adults, who meet eligibility requirements, with assistance in navigating, selecting and coordinating services. Under the program, a trained service coordinator meets with a participant to determine that individual’s needs. The coordinator will then develop a plan to set up and organize those services, with the goal of keeping that individual in his or her home.

Eligibility participants for the Aging Waiver Service Coordination Program must:
- Be a resident of Pennsylvania;
- Be a U.S. citizen or a qualified non-citizen;
- Have a Social Security number;
- Be 60 years of age or older;
- Meet the level of care needs for a skilled nursing facility; and,
- Meet the financial requirements as determined by your local County Assistance Office.
A service coordinator will set up an initial meeting with a program participant (and, if desired, involving, additional family, friends or other caretakers), to discuss what services are currently provided and what help is needed to allow the individual to live in his or her home. This discussion may also include recommendations and referrals for other benefits for which a participant may be eligible. These services can include:
- Personal care services, such as bathing or dressing
- Home modification
- Meal delivery or preparation
- Adult day services, such as coordinating transportation
Afterward, the service coordinator will assist with coordination of the needed services. Participants of the Aging Waiver have the ability to select providers and services according to their own preferences. The service coordinator will regularly contact the participant to determine if the services being received continue to meet the needs of the individual.

AAA and the Aging Waiver
Area Agency on Aging (AAA) was the first agency in Allegheny County to provide service coordination in the Aging Waiver. For more than 16 years, the AAA has provided service under the Pennsylvania Aging Waiver to Older Adults. The continued goal is to help older adults and their families find the services needed to help them stay in their homes. In addition to the Aging Waiver, the AAA has a broad array of programs that may compliment services received through the Aging Waiver.
